Social Prescribing is now Live!

Social prescribing is a means of enabling GPs, nurses and other primary care professionals to refer people to a range of local, non-clinical services. Recognising that people's health is determined by a range of social, economic and environmental factors, social prescribing seeks to address people's needs in a holistic way. It also aims to support individuals to take greater control of their own health.
In August we launched our Wellbeing Menu; a handy directory of local groups and services that anyone can use. As part of the Kenilworth and Warwick Network we have now taken on Amanda Jerman as our network social prescriber. Amanda will be here at Chase Meadow Health Centre on Thursday afternoons to see patients and develop social prescribing across our network of Practices. You can self refer into the service by completing this form.
